What is the Chronic Care Management Program?

For many people, chronic health conditions are a constant battle. Heart Disease, stroke, cancer, type 2 diabetes, obesity, and arthritis are some of the most common chronic health issues that arise. One in four adults has two or more chronic health conditions, and seven of the top 10 causes of death in 2010 were due to chronic diseases.

The Chronic Care Management Program

We want to make sure we are looking out for our patients affected by these conditions and that is why we offer the Chronic Care Management Program. 

This program is designed to keep our patients on top of their conditions. As part of the program, we develop a patient- centered care plan that includes:

  • Measurable treatment goals
  • Symptom management
  • Medication management
  • Community and social services 

We Come Prepared – And Offer On-Going Care

Our staff is well versed in all our patient’s medical needs prior to enrolling them in the program so that they can offer the best course of action on a patient to patient basis. We want to make sure that each of our patients is getting a personalized plan, ensuring their success. 

After enrolling patients up in this program, we offer on-going care. Overseeing things like: 

  • Medication self-management
  • Updating the care plan based off monthly phone calls 
  • Educating family members and caretakers on topics like diet, exercise, and medications

Continuing Assistance For Appointments & More

We also help with any appointments needed with specialists or primary care doctors. If there are additional services a patient needs, such as community and health resources, we help find those as well.

This program has helped many patients get access to things they may not have been aware of or been able to get before, such as mental health services, in home nursing aids, and physical therapy. Access to these services makes it easier for patients to manage their chronic health issues, which can help increase their quality of life.
